Clinical vignette

In May 2009, a 66-year-old woman with a past medical history of hyperlipidemia and breast cancer is referred for seasonal allergic rhinitis and oral allergy syndrome. She also reports previous allergic reactions to penicillin and trimethoprim-sulfamethoxazole. At age 22 years, she received intramuscular penicillin for streptococcal pharyngitis and had immediate symptoms of generalized pruritic urticaria, dyspnea, wheezing, and dizziness. She was treated with injectable epinephrine and rapidly

Moreover, IgE expression is significantly increased in children with MPP or MP-related diseases [22–24]. In addition, overproduction of IgE plays an important role in the pathogenesis of allergic diseases [25]. A clinical symptom of an allergic response is IgE antibodies secreted by plasmacytes, and these IgE antibodies can bind to basophils and mast cells, which become sensitized.

This measure could be an upper bound while the true percentage of true hypersensitivity may be lower. Although it is reported that skin tests have high negative predictive values,9 it is noteworthy that negative predictive values are affected by the prevalence of disease and are expected to be high in children reporting amoxicillin reaction given the low prevalence of true amoxicillin hypersensitivity in this population.7,8 Therefore, to validate tests, it is crucial to assess the sensitivity of the test compared with the criterion standard, that is, drug challenge.

However, we did not perform multivariate analysis because there were not enough events for multivariate analysis. Notwithstanding, a long OA administration period may lead to the development of AMR and other side effects, such as various allergies [33–36]; thus, administration for a shorter period is desirable. It is speculated that the results of this study would be useful in many respects because shortening the administration of OA also affects medical cost reduction and compliance.
