En niños menores de un año, los traumatismos arteriales se deben fundamentalmente a la yatrogenia preoperatoria. En los últimos 10 años se han atendido 11 pacientes menores de un año con lesiones arteriales originadas por punción deliberada, habiendo necesitado tratamiento quirúrgico, intravascular o mixto. El seguimiento intra y postoperatorio de las malformaciones congénitas graves implica la necesidad de tomar un acceso arterial adecuado. Éste se tomará mediante rigurosas normas de actuación: limitando el número de punciones realizadas por personal con experiencia y sólo sobre troncos principales. Tras la lesión el abordaje endovascular primero y quirúrgico a continuación será necesario a corto plazo en la mayoría de los casos. En conclusión, el cuidado en los accesos arteriales y su mantenimiento constituyen la mejor terapia preventiva. Las medidas adecuadas y precoces ante fístulas, seudoaneurismas y trombosis pueden evitar complicaciones dramáticas, especialmente en pacientes de bajo peso

Traumatic vascular lesions in neonates are usually due to preoperative iatrogeny. In the last ten years, we attended 11 patients aged less than one year with arterial lesions due to deliberate puncture. All required endovascular, surgical or combined treatment. The intra- and postoperative follow-up of severe congenital malformations necessitates appropriate arterial access. This should be achieved following strict protocols, with a limited number of punctures carried out by experienced staff and only in large arteries. When a vascular lesion occurs, first an endovascular and then a surgical approach is required in the shortest time period possible. In conclusion, care of arterial routes of access and their maintenance is the best preventive therapy. Appropriate and early treatment in fistulas, pseudoaneurysms and thrombosis must be provided to prevent serious complications, especially in underweight patients
