Objetivo

Evaluar el estado de salud y nutrición de mujeres adolescentes delincuentes en el momento de su ingreso en un centro de reforma juvenil.

Métodos

Estudio retrospectivo transversal durante 6 años (1995-2000). Se evaluó el estado de salud por historia clínica y exploración física siguiendo protocolos estandarizados, realizando exámenes complementarios de laboratorio de forma individualizada. Se evaluó el estado nutricional mediante método antropométrico, determinando peso, talla, perímetros cefálico y braquial, índice de masa corporal e índice nutricional y se compararon con estándares nacionales normales de referencia (Z-score), valorando la existencia de diferencias estadísticamente significativas (test de la t de Student no apareado).

Resultados

Durante el período de estudio ingresaron 35 mujeres adolescentes delincuentes con una edad media de 15 años (0,72 desviaciones estándar [DE]) (límites, 14-17 años). Los problemas de salud más frecuentemente observados fueron: tabaquismo (n = 32), abuso de drogas/alcohol (n = 22), conductas sexuales de alto riesgo (n = 14), trastornos psicopatológicos (n = 12), enfermedades odontológicas (n = 11), enfermedades infecciosas relacionadas con el consumo parenteral de drogas y/o las actividades sexuales de riesgo (n = 10) (vaginitis específica [n = 3], vaginitis bacteriana [n = 2], sida [n = 2], hepatitis C [n = 2], hepatitis B [n = 1]) y estado de inmunización incompleta (n = 6). En menor número se objetivaron anemia ferropénica (n = 3), test de embarazo positivo (n = 2), trastornos dermatológicos (n = 2), oftalmológicos (n = 2) y neurosensoriales (n = 1). No se objetivaron trastornos de los sistemas respiratorio, locomotor, otorrinolaringológico, cardiovascular y digestivo. El análisis de los parámetros e índices antropométricos evaluados, a nivel individual y en el grupo, no objetivó la existencia de trastornos de crecimiento y nutrición.

Conclusiones

En nuestro entorno, las mujeres adolescentes delincuentes llegan al centro de reforma con sustanciales problemas físicos, psicológicos y de conducta preexistentes. Esta población está especialmente necesitada de servicios psiquiátricos, odontológicos, ginecológicos, infecciosos y de inmunización. El tiempo en custodia representa una oportunidad única para dirigir los aspectos básicos de salud de esta población de alto riesgo y proporcionar educación sanitaria general que incluya educación sobre abuso de sustancias y educación sexual.

Objective

To assess the health and nutritional status of delinquent female adolescents on admission to a juvenile correctional facility.

Methods

:We performed a cross-sectional study over a 6-year period (1995-2000). Health status was assessed through medical history and physical examination according to standard protocols and individualized complementary laboratory examinations. Nutritional status was assessed using the anthropometric method. Height, weight, head and upper arm circumferences, body-mass index and nutritional index were determined and compared with normal national standards of reference (Z-score). Student’s single-sample t-test was used to assess statistically significant differences.

Results

Thirty-five delinquent female adolescents were admitted during the study period. Mean age was 15 years (0.72 SDU) (range: 14-17 years). The most frequently observed health problems were smoking (n 5 32), drug/alcohol abuse (n 5 22), high-risk sexual behavior (n 5 14), psychopathologic disorders (n 5 12), odontologic diseases (n 5 11), infectious diseases associated with intravenous drug use and/or high-risk sexual behavior (n 5 10) [specific vaginitis (n 5 3), bacterial vaginitis (n 5 2), AIDS (n 5 2), hepatitis C (n 5 2), hepatitis B (n 5 1)] and incomplete immunization status (n 5 6). Less prevalent health problems were iron deficiency anemia (n 5 3), positive pregnancy test (n 5 2), and dermatological (n 5 2), ophthalmological (n 5 2) and neurosensorial (n 5 1) disorders. No respiratory, locomotor, otorhinolaryngological, cardiovascular or gastrointestinal disorders were observed. Individual and group analysis of established anthropometric parameters and indices showed no growth or nutritional disorders.

Conclusions

In our environment, delinquent female adolescents arrive at correctional facilities with significant physical, psychological and behavioral problems. This population is especially in need of psychiatric, dental, gynecologic, infectious, and immunization services. The time in custody presents a unique opportunity to address the basic health concerns of this high-risk population and to provide general health education, including education on substance abuse and sex education.
