    "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introducci&#243;n</span><p id="par0005" class="elsevierStylePara elsevierViewall">Los avances de la Neonatolog&#237;a en las &#250;ltimas d&#233;cadas han conseguido disminuir significativamente la mortalidad de los ni&#241;os nacidos con peso menor de 1&#46;500<span class="elsevierStyleHsp" style=""></span>g o con una edad gestacional menor de 32 semanas &#40;&#60;1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32&#41;&#46; Dado el mayor riesgo de discapacidad en los<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32&#44; el seguimiento tras el alta se considera una actividad crucial de los cuidados neonatales&#46; En algunos pa&#237;ses&#44; el desarrollo de un programa de seguimiento es requisito imprescindible para que un centro reciba la acreditaci&#243;n para formar especialistas en Neonatolog&#237;a<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">1</span></a>&#46; A pesar de todo ello&#44; se reconoce que&#44; en general&#44; faltan gu&#237;as cl&#237;nicas o protocolos consensuados a nivel nacional para estandarizar el seguimiento de estos ni&#241;os<a class="elsevierStyleCrossRefs" href="#bib0410"><span class="elsevierStyleSup">1&#8211;4</span></a>&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">Se considera que la edad m&#237;nima de seguimiento son los 2 a&#241;os de edad corregida &#40;EC&#41;&#46; Aunque tambi&#233;n se asume que de esta forma se detecta principalmente la discapacidad moderada&#47;grave y que queda sin identificar&#44; y sin orientar&#44; muchas de las secuelas que se muestran de forma m&#225;s tard&#237;a en estos ni&#241;os y que tienen que ver fundamentalmente con los aspectos emocionales&#44; del comportamiento y del aprendizaje&#46; Por tanto&#44; ser&#237;a deseable que los programas de seguimiento se desarrollaran para prestar atenci&#243;n hasta los 5-7 a&#241;os de edad&#44; o incluso hasta el final de la edad pedi&#225;trica<a class="elsevierStyleCrossRef" href="#bib0425"><span class="elsevierStyleSup">4</span></a>&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">Por todos los motivos anteriormente expuestos&#44; y dado que no hay protocolos nacionales disponibles sobre seguimiento de<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32&#44; la Sociedad Espa&#241;ola de Neonatolog&#237;a&#44; en colaboraci&#243;n con la Asociaci&#243;n Espa&#241;ola de Pediatr&#237;a de Atenci&#243;n Primaria&#44; propone un protocolo de seguimiento espec&#237;fico para los<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32&#44; entendiendo como protocolo una gu&#237;a de acciones comunes que busca estandarizar las actividades seg&#250;n los criterios de buena pr&#225;ctica&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">Las necesidades de los<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32 durante los a&#241;os de seguimiento son muy diversas y&#44; en muchos casos&#44; complejas&#46; El protocolo solo se centra en los aspectos que son aplicables a todos los ni&#241;os durante su seguimiento&#44; independientemente de d&#243;nde se realicen las revisiones&#46; Adem&#225;s&#44; por supuesto&#44; en funci&#243;n de los problemas que presenten el ni&#241;o y la familia&#44; se deber&#225;n prestar otro tipo de cuidados que no quedan recogidos en el protocolo&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Los objetivos de este protocolo son&#58;<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">&#8226;</span><p id="par0030" class="elsevierStylePara elsevierViewall">Determinar y estandarizar las evaluaciones que se deber&#237;an realizar a los ni&#241;os<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32&#44; El desarrollo de los programas de seguimiento es un elemento esencial de las Unidades Neonatales de nivel asistencial 3&#44; en colaboraci&#243;n con otros profesionales<a class="elsevierStyleCrossRefs" href="#bib0410"><span class="elsevierStyleSup">1&#44;5&#44;6</span></a> &#40;fuerza de la recomendaci&#243;n B&#46; Este aspecto es b&#225;sico para coordinar los recursos&#44; evitar duplicidades&#44; aumentar en eficiencia y eficacia&#44; actuar con premura ante las necesidades del beb&#233; y la familia y explotar los datos para la evaluaci&#243;n de la pr&#225;ctica cl&#237;nica o para otros estudios &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li><li class="elsevierStyleListItem" id="lsti0055"><span class="elsevierStyleLabel">&#8226;</span><p id="par0090" class="elsevierStylePara elsevierViewall">Se recomienda que todos los ni&#241;os<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32 dados de alta hospitalaria sean valorados por el pediatra de AP en un tiempo nunca superior a los 5 d&#237;as despu&#233;s del alta<a class="elsevierStyleCrossRef" href="#bib0475"><span class="elsevierStyleSup">14</span></a> &#40;fuerza de la recomendaci&#243;n B&#41;&#46; Para ello es deseable que se tramite la tarjeta sanitaria individual antes del alta y se asigne el pediatra y enfermera de AP&#46;</p></li><li class="elsevierStyleListItem" id="lsti0060"><span class="elsevierStyleLabel">&#8226;</span><p id="par0095" class="elsevierStylePara elsevierViewall">En situaci&#243;n de riesgo socioambiental&#44; se recomienda que el equipo de AP confirme la implicaci&#243;n de los trabajadores sociales y se coordine con ellos &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li></ul></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Recomendaciones relacionadas con el crecimiento</span><p id="par0100" class="elsevierStylePara elsevierViewall"><ul class="elsevierStyleList" id="lis0015"><li class="elsevierStyleListItem" id="lsti0065"><span class="elsevierStyleLabel">&#8226;</span><p id="par0105" class="elsevierStylePara elsevierViewall">Se recomienda monitorizar el crecimiento tras el alta&#46; El crecimiento insuficiente se asocia con problemas en el neurodesarrollo&#46; La ganancia excesiva de peso se asocia con mayor riesgo de obesidad en la edad adulta&#44; enfermedad cardiovascular y diabetes<a class="elsevierStyleCrossRefs" href="#bib0480"><span class="elsevierStyleSup">15&#8211;21</span></a>&#46; El objetivo deseable es que en los 3<span class="elsevierStyleHsp" style=""></span>primeros meses tras el alta se alcance una ganancia ponderal de 25-35<span class="elsevierStyleHsp" style=""></span>g&#47;d&#237;a &#40;fuerza de recomendaci&#243;n B&#41;&#46;</p></li><li class="elsevierStyleListItem" id="lsti0070"><span class="elsevierStyleLabel">&#8226;</span><p id="par0110" class="elsevierStylePara elsevierViewall">Para la evaluaci&#243;n del crecimiento tras el alta se recomienda el uso de las curvas de Fenton revisadas hasta las 44 semanas de EC y posteriormente las curvas de la Organizaci&#243;n Mundial de la Salud &#40;OMS&#41; de acuerdo con la EC<a class="elsevierStyleCrossRefs" href="#bib0515"><span class="elsevierStyleSup">22&#44;23</span></a> &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li><li class="elsevierStyleListItem" id="lsti0075"><span class="elsevierStyleLabel">&#8226;</span><p id="par0115" class="elsevierStylePara elsevierViewall">Se recomienda que los ni&#241;os de bajo peso para la edad gestacional que a los 2-3 a&#241;os tengan una talla por debajo de 2 desviaciones est&#225;ndar en las gr&#225;ficas de la OMS se remitan al endocrin&#243;logo para completar el estudio y valorar el tratamiento con hormona de crecimiento<a class="elsevierStyleCrossRef" href="#bib0525"><span class="elsevierStyleSup">24</span></a> &#40;fuerza de recomendaci&#243;n B&#41;&#46;</p></li></ul></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Recomendaciones relacionadas con la alimentaci&#243;n<a class="elsevierStyleCrossRef" href="#bib0530"><span class="elsevierStyleSup">25</span></a></span><p id="par0120" class="elsevierStylePara elsevierViewall"><ul class="elsevierStyleList" id="lis0020"><li class="elsevierStyleListItem" id="lsti0080"><span class="elsevierStyleLabel">&#8226;</span><p id="par0125" class="elsevierStylePara elsevierViewall">Se recomienda que el ni&#241;o se alimente con leche materna de forma exclusiva hasta los 6 meses y posteriormente reciba otros alimentos complementarios&#44; hasta los 2 a&#241;os o hasta que la madre y el ni&#241;o deseen<a class="elsevierStyleCrossRef" href="#bib0535"><span class="elsevierStyleSup">26</span></a>&#46; Se ha mostrado que la leche materna previene enfermedades durante el ingreso hospitalario<a class="elsevierStyleCrossRefs" href="#bib0540"><span class="elsevierStyleSup">27&#8211;30</span></a> y mejora el desarrollo cognitivo<a class="elsevierStyleCrossRefs" href="#bib0560"><span class="elsevierStyleSup">31&#44;32</span></a>&#46; No existen estudios centrados en el valor de la leche materna tras el alta en este grupo espec&#237;fico de ni&#241;os &#40;fuerza de la recomendaci&#243;n B&#41;&#46;</p></li><li class="elsevierStyleListItem" id="lsti0085"><span class="elsevierStyleLabel">&#8226;</span><p id="par0130" class="elsevierStylePara elsevierViewall">Dado que la composici&#243;n de la leche materna es muy variable&#44; el volumen de leche que precisa un ni&#241;o para alcanzar el aporte cal&#243;rico adecuado es tambi&#233;n variable&#46; Por ello se recomienda que los ni&#241;os con riesgo nutricional &#40;que no hayan alcanzado el P10 al alta&#44; precisen restricci&#243;n h&#237;drica o cuya ganancia ponderal con lactancia materna exclusiva no sea correcta o presenten osteopenia de la prematuridad&#41; reciban leche materna suplementada con fortificantes en alguna toma o bien se recomienda sustituir alguna toma por f&#243;rmula de prematuros &#40;siempre despu&#233;s de haber verificado que la t&#233;cnica de lactancia materna es correcta&#41;&#46; Los estudios no han mostrado diferencias entre la administraci&#243;n de fortificantes o de f&#243;rmula<a class="elsevierStyleCrossRefs" href="#bib0570"><span class="elsevierStyleSup">33&#8211;35</span></a> &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li><li class="elsevierStyleListItem" id="lsti0090"><span class="elsevierStyleLabel">&#8226;</span><p id="par0135" class="elsevierStylePara elsevierViewall">Si se administran fortificantes a ni&#241;os que est&#225;n mamando&#44; la pauta m&#225;s recomendada actualmente es diluir los 4<span class="elsevierStyleHsp" style=""></span>o 5<span class="elsevierStyleHsp" style=""></span>g del fortificante &#40;en funci&#243;n del fabricante&#41; en unos 30-50<span class="elsevierStyleHsp" style=""></span>ml de leche materna extra&#237;da y administr&#225;rselo al ni&#241;o antes de la toma 2<span class="elsevierStyleHsp" style=""></span>o 3<span class="elsevierStyleHsp" style=""></span>veces al d&#237;a<a class="elsevierStyleCrossRef" href="#bib0585"><span class="elsevierStyleSup">36</span></a>&#46; Se debe evitar a&#241;adir a esa leche otros suplementos&#44; como vitaminas y hierro&#46; No se recomienda pasar de 15<span class="elsevierStyleHsp" style=""></span>g&#47;d&#237;a de fortificante porque existe riesgo de hipervitaminosis A&#46; Se debe ajustar la dosis de vitamina D cuando est&#225;n tomando fortificante &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li><li class="elsevierStyleListItem" id="lsti0095"><span class="elsevierStyleLabel">&#8226;</span><p id="par0140" class="elsevierStylePara elsevierViewall">La producci&#243;n de leche durante las primeras semanas tras el alta se puede incrementar con el m&#233;todo canguro en el domicilio&#59; por ello&#44; se recomienda que todas las madres&#44; en el momento del alta&#44; sepan c&#243;mo poner a su hijo en posici&#243;n canguro&#46; Numerosos estudios han mostrado este efecto positivo del canguro en relaci&#243;n con la producci&#243;n de leche&#44; aunque la mayor parte de ellos son intrahospitalarios<a class="elsevierStyleCrossRefs" href="#bib0590"><span class="elsevierStyleSup">37&#44;38</span></a> &#40;fuerza de la recomendaci&#243;n B&#41;&#46;</p></li><li class="elsevierStyleListItem" id="lsti0100"><span class="elsevierStyleLabel">&#8226;</span><p id="par0145" class="elsevierStylePara elsevierViewall">Existe gran controversia entre el uso tras el alta de f&#243;rmula adaptada habitual &#40;65<span class="elsevierStyleHsp" style=""></span>kcal&#47;dl&#41; o f&#243;rmulas de prematuros &#40;75-80<span class="elsevierStyleHsp" style=""></span>kcal&#47;dl&#41;<a class="elsevierStyleCrossRefs" href="#bib0600"><span class="elsevierStyleSup">39&#8211;41</span></a>&#46; La Sociedad Europea de Gastroenterolog&#237;a&#44; Hepatolog&#237;a y Nutrici&#243;n Pedi&#225;trica &#40;ESPGHAN&#41; recomienda el uso de f&#243;rmula de prematuros hasta la semana 40 de EC e incluso hasta m&#225;s tarde si el crecimiento es sub&#243;ptimo<a class="elsevierStyleCrossRef" href="#bib0615"><span class="elsevierStyleSup">42</span></a>&#46; La OMS recomienda su uso solo hasta los 2&#46;000<span class="elsevierStyleHsp" style=""></span>g<a class="elsevierStyleCrossRef" href="#bib0620"><span class="elsevierStyleSup">43</span></a>&#46; No se puede realizar ninguna recomendaci&#243;n en este sentido&#46; En caso de administrarse f&#243;rmula de prematuros de forma exclusiva&#44; si se mantiene m&#225;s all&#225; de la semana 40 de EC&#44; hay que intentar retirarla cuanto antes porque el aporte de f&#243;sforo y calcio resulta excesivo<a class="elsevierStyleCrossRef" href="#bib0625"><span class="elsevierStyleSup">44</span></a>&#46; S&#237; que se puede administrar alguna toma de f&#243;rmula de prematuros m&#225;s all&#225; de la semana 40 si el ni&#241;o est&#225; con lactancia materna o con f&#243;rmula de inicio y se quiere aumentar el aporte cal&#243;rico &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li><li class="elsevierStyleListItem" id="lsti0105"><span class="elsevierStyleLabel">&#8226;</span><p id="par0150" class="elsevierStylePara elsevierViewall">La introducci&#243;n de la alimentaci&#243;n complementaria debe considerar las capacidades motoras del ni&#241;o en relaci&#243;n con la alimentaci&#243;n<a class="elsevierStyleCrossRefs" href="#bib0630"><span class="elsevierStyleSup">45&#44;46</span></a>&#44; pero no existen estudios que demuestren las ventajas o desventajas de guiar la alimentaci&#243;n en funci&#243;n de edad real o EC&#58; deber&#237;a individualizarse &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li></ul></p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Recomendaciones relacionadas con el desarrollo motor</span><p id="par0155" class="elsevierStylePara elsevierViewall"><ul class="elsevierStyleList" id="lis0025"><li class="elsevierStyleListItem" id="lsti0110"><span class="elsevierStyleLabel">&#8226;</span><p id="par0160" class="elsevierStylePara elsevierViewall">Aunque aparentemente el desarrollo sea adecuado&#44; se recomienda una detenida evaluaci&#243;n motora por Neurolog&#237;a al menos 2<span class="elsevierStyleHsp" style=""></span>veces en el primer a&#241;o de vida<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">1</span></a>&#46; Los 3 meses de EC son un buen momento para la valoraci&#243;n de los movimientos de Prechtl&#44; con una alt&#237;sima capacidad predictiva en cuanto a la funci&#243;n motora posterior<a class="elsevierStyleCrossRefs" href="#bib0640"><span class="elsevierStyleSup">47&#44;48</span></a>&#46; Servir&#225; para identificar a los ni&#241;os que probablemente van a tener problemas motores y&#44; por tanto&#44; pueden ser enviados a Rehabilitaci&#243;n&#44; porque son los que m&#225;s van a beneficiarse de una intervenci&#243;n temprana&#46; La segunda evaluaci&#243;n se puede realizar en torno a los 12 meses de EC &#40;fuerza de la recomendaci&#243;n B&#41;&#46;</p></li><li class="elsevierStyleListItem" id="lsti0115"><span class="elsevierStyleLabel">&#8226;</span><p id="par0165" class="elsevierStylePara elsevierViewall">Todos los ni&#241;os<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>1&#46;500<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>32 tienen un riesgo incrementado de alteraciones motoras&#44; por tanto&#44; se recomienda remitirlos a Atenci&#243;n Temprana porque con los programas de intervenci&#243;n precoz se ha mostrado mejor&#237;a tanto en la funci&#243;n cognitiva como motora<a class="elsevierStyleCrossRefs" href="#bib0650"><span class="elsevierStyleSup">49&#44;50</span></a> &#40;fuerza de la recomendaci&#243;n A&#41;&#46;</p></li><li class="elsevierStyleListItem" id="lsti0120"><span class="elsevierStyleLabel">&#8226;</span><p id="par0170" class="elsevierStylePara elsevierViewall">Es recomendable que&#44; en los ni&#241;os con problemas motores&#44; la funci&#243;n motora y sus capacidades se describan utilizando las diversas escalas que hay disponibles para ellos&#46; De esta forma&#44; se puede valorar de una forma objetiva la evoluci&#243;n del ni&#241;o&#59; asimismo&#44; se facilita la comparaci&#243;n entre centros&#44; al permitir una clasificaci&#243;n m&#225;s homog&#233;nea &#40;posicionamiento I<span class="elsevierStyleSmallCaps">&#41;</span>&#46;</p></li></ul></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Recomendaciones respecto al desarrollo neuropsicol&#243;gico</span><p id="par0175" class="elsevierStylePara elsevierViewall"><ul class="elsevierStyleList" id="lis0030"><li class="elsevierStyleListItem" id="lsti0125"><span class="elsevierStyleLabel">&#8226;</span><p id="par0180" class="elsevierStylePara elsevierViewall">El m&#233;todo madre canguro&#44;
